Coverage
The physician community has struggled with burdensome guidelines for reporting office visits and other E/M codes. As a result, electronic health records have inadvertently made it easy to add extraneous information to the patient record. To remedy this issue, the CPT Editorial Panel approved major revisions to the E/M office or other outpatient visit reporting guidelines and their code descriptions.
